Request Information

Choose the appropriate report type below:

  1. Medical Report Request - Request a copy of your ambulance medical record.
  2. Incident Report - Request an incident report related to a fire, vehicle collision, or another incident. For incidents involving medical care, please refer to “Medical Report Request” section above.
  3. Public Records Act Requests (Other)  - If your request does not fall under the categories listed above, you may submit a general Public Records Act (PRA) request. Before submitting a PRA, please ensure none of the other report types meet your needs. 

1. Medical Report Request

All medical records requests are managed by our billing partner, Wittman Enterprises. To request a copy of your record, please contact Wittman Enterprises directly at 800-906-6552

2. Incident Report

Request a report related to a fire, vehicle collision, or other non-medical incident. For incidents involving medical care, please submit a request through the “Medical Report Request” section. 
 
 Requests are typically processed within 10 calendar days. Please note that incomplete or inaccurate information may delay processing or prevent the report from being located. You may choose to: 
  •  Pick up the report in person – You will be contacted by email when the report is ready. Pick-up is available at the American Canyon Fire Protection District, located at: 911 Donaldson Way East, American Canyon, CA 94503. Fees will apply for printed copies. 
  • Receive the report by mail – A fee may apply. You will be notified when your request has been processed and informed of any applicable charges. 

3. Public Records Act Requests (Other)

If your request does not fall under the categories listed above, you may submit a general Public Records Act (PRA) request. Requests are typically processed within 10 calendar days. Please ensure that all information provided is complete and accurate, as missing or incorrect details may delay processing or prevent us from locating the requested report. You may choose to receive your report in one of the following ways:

  • Pick Up in Person – You will receive an email notification when your report is ready. Reports can be picked up at the American Canyon Fire Protection District: 911 Donaldson Way East, American Canyon, CA 94503. A fee will apply for printed copies. 

  • Receive by Mail – A fee may apply. Once your request is processed, you will be notified of any applicable charges before the report is mailed. 

Note:  The American Canyon Fire Protection District does not maintain environmental records such as underground storage tank data and hazardous material storage/history. Please contact Napa County Environmental Management; at (707) 253-4471 for assistance with those records.
